Housing and Services Coordination Task Force Task Force
GOALS & PROJECTS
Address the 10 year plan of Access to Services goal, speciffically focusing on Case Management and Serivce Provision. The three main areas to be address in this task foce are:
Training and standardizing case management and services
Providing advocacy about the needs of the community, participants, services, and resources
Breaking down barriers to housing, employment, and other systems.
ACHIEVEMENTS This task force most recently created the housing resource guide, which assists providers who are looking for a permanent housing option for their clients.
The task force also took a lead role in the creation of the Metro Home BASE triage tool and common assessment tool.
